Due to the introduction of electronic consent, please see attached leaflet and letter from the Health Services for Children and Young People at Nottinghamshire Healthcare NHS Foundation Trust regarding flu immunisation for your child at school.
Please read and follow the instructions carefully to consent / decline flu immunisation for your child - due to take place in school on 18.10.19.
